Sulos LLC (“Sulos”) understands the importance of user privacy and is committed to protecting your privacy. The purpose of this privacy policy (“Privacy Policy”) is to explain our practices regarding collection, use, and disclosure of any information and the choices you can make about the way your information is collected and used, whether via our website at https://www.sulos.org/ (the “website” or “Site”), any software or mobile applications (“App(s)”) or via other online submissions by you or authorized third parties to Sulos. This Privacy Policy supplements and is incorporated as a part of Sulos’s Terms of Use available at https://www.sulos.org/terms-of-use and the End User License Agreement at https://www.sulos.org/user-agreement.
By installing, using or accessing the Apps or the Site, you understand and agree to the terms herein. If you do not agree to this Privacy Policy or the Terms of Use, do not use or access the Apps or Site. Please delete the App from all the devices on which the App was installed.
The contents and information provided on the Site and App (“Content”) are for informational purposes only. The Content is not intended to be a substitute for professional medical advice, diagnosis, or treatment. Collection of Your Personal Information
When you visit the Site, certain kinds of information, such as the website that referred you to us, your IP address, browser type and language, and access times, may be collected automatically as part of the Site’s operation. We also may collect navigational information, including information about the pages you view, the links you click, and other actions taken in connection with the Site and Apps. When you register for an account, Personal Information is collected. Personal Information means any information that may be used to identify an individual, including, but not limited to your first and last name, your mailing address, email address, phone number, social security number, or insurance ID number. We may combine your visit and navigational information with Personal Information that you provide. You may always choose not to provide Personal Information, but if you so choose, certain products and services may not be available to you. Confidentiality of Health Information
Some of the users of the Site and Apps – such as healthcare providers or caregivers – are subject to laws and regulations governing the use and disclosure of personal health information that they create or receive from other sources. Included among those laws is the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). When Sulos stores, processes or transmits “individually identifiable health information” (as that term is defined in HIPAA) on behalf of a healthcare provider who has entered into a user agreement with us, we do so as the provider’s “business associate,” as that term is defined in HIPAA. As a business associate, Sulos is prohibited from using individually identifiable health information to the same extent that the healthcare provider itself is prohibited from using it. Sulos is required to take appropriate measures to safeguard the confidentiality, integrity and availability of individually identifiable health information which it collects, stores and processes on behalf of such providers.
Use of Your Personal Information
Sulos collects and may use your Personal Information to operate and improve our websites, the Apps, to process your transactions, to provide customer service or troubleshoot, to perform research and analysis aimed at improving our products, services and technologies, and to display content that is customized to your preferences. In certain cases, we collect and use your Personal Information to comply with laws and regulations.
We also use your Personal Information to communicate with you. We may send transaction-related communications such as welcome messages and confirmations. We may also send you surveys or marketing communications to inform you of new products or services or other information that may be of interest. If you do not wish to receive marketing communications, you may adjust your “Preferences” or follow the “unsubscribe” instructions included within each email communication. Please keep in mind that, if you choose not to receive marketing communications, you will continue to receive transactional or account communications (e.g., confirmation and verification emails).
Personal Information collected by Sulos may be stored and processed in the United States or any other country in which Sulos or its affiliates, subsidiaries or agents maintain facilities, and by using our sites or services, you consent to any such transfer of information outside of your country.
Session and Usage Data
The services provided on the Site or Apps collect certain information from you when you share that information with us and when you use the services. Our Apps may collect information about your device and how it is being used. This includes the make and model of your phone, your mobile operating system and the type of browser you are using.
We may link the information we record using tracking technologies to your Personal Information we collect at the Site for our own and our marketing partners’ marketing and advertising purposes. In addition, we reserve the right to use IP addresses and other tracking technologies to identify a visitor when we feel it is necessary to enforce compliance with our Terms of Use, to protect the Site, our customers or others, or when we believe in good faith that the law requires it.
Sharing of Your Personal Information
Except as disclosed in this Privacy Policy, Sulos does not share your Personal Information with any outside parties. We may share your Personal Information with service providers who perform services on our behalf. For example, we may hire other companies to handle the processing of payments, to provide data storage, to host websites, to assist in direct marketing, to conduct audits, etc. Those companies will be permitted to obtain only the Personal Information they need to provide the service. They are required to maintain the confidentiality of the information and are prohibited from using it for any other purpose. Information about our customers, including Personal Information, may be disclosed as part of any merger, acquisition, or sale of the company and/or its assets, as well as in the unlikely event of insolvency, bankruptcy, or receivership, in which Personal Information would be transferred as one of the business assets of the company. We will notify you of such an occurrence as described in the “Changes to This Privacy Policy” section below. We reserve the right to disclose your Personal Information if required to do so by law, or in the good-faith belief that such action is reasonably necessary to comply with legal process, respond to claims, or protect the rights, property or safety of our company, employees, customers, or the public.
Accessing Your Personal Information; Preferences

We may require proof of your identity before we edit or provide you with the requested information. We will use reasonable efforts to supply you with this information and to correct any factual inaccuracies that you identify. Please also contact us if you do not want us to disclose your Personally Information to third parties or you want us to delete your Personally Information from our databases. Keep in mind, however, that there may be residual information within our databases, access logs and other records, which may or may not contain such information. The residual information will not be used for commercial purposes; however, we reserve the right, from time to time, to re-contact former users of the services. We allow you to challenge the data that we hold about you and, where appropriate, you may have the data erased, rectified, amended, or completed.
Certain users such as healthcare providers may be required under HIPAA and other applicable laws to retain your information for extended periods of time, and Sulos will continue to retain such information on their behalf in accordance with such requirements. Users should submit requests to access or correct their health information directly to their providers.
Use of Cookies and Other Technologies
Sulos uses “cookies” to enable you to sign in to our services and to help personalize your online experience. A cookie is a small text file that is placed on your hard drive. Cookies contain information, including Personal Information, that can later be read by a web server in the domain that issued the cookie to you. The information that cookies collect includes the date and time of your visit, your registration information, and your navigational and purchase history. Cookies offer you many conveniences. They allow us to identify registered users when they return to the site so that they can retrieve previous image search results. Cookies also save you time by eliminating the need to repeatedly enter the same information. In some cases, our third-party service providers may use cookies on our sites. We have no access to or control over these cookies. This Privacy Policy covers the use of cookies by Sulos only, and does not cover the use of cookies by third parties. You have the ability to accept or decline cookies. Most browsers automatically accept cookies, but you can usually modify your browser setting to decline cookies. If you choose to decline cookies, you may not be able to sign in or use other interactive features of our sites and services that depend on cookies. In addition to cookies, we may use other technologies, including single-pixel gifs (also known as web beacons) on our websites and in promotional e-mail messages or newsletters. These tiny electronic images assist us in determining how many users have visited certain pages or opened messages or newsletters. We do not use these images to collect Personal Information.
Security of Your Personal Information
The security of your Personal Information is important to us. We take reasonable precautions to protect the privacy, accuracy and reliability of any information you provide and to protect such information from loss, misuse, unauthorized access, disclosure, alteration and destruction. Personally Information you provide and we collect is stored within databases that are controlled by us or by our service providers. As we deem appropriate, we use security measures consistent with industry standards, such as firewalls and encryption technology, to protect your information. However, since no security system is impenetrable, we cannot guarantee the security of our databases, nor can we guarantee that information you supply will not be intercepted while being transmitted to us over the Internet. As a result, you transmit any information to or from the Site/Apps at your own risk.
If you create an account to use the Site/Apps, you may be asked to choose a password for your account, in which case your online account information shall be protected by your password. You are responsible for maintaining the confidentiality of your password and user account and are fully responsible for all activities that occur under your password or account. You can access and update information in your account profile at any time by logging in to the website with your login name and password. Once you have logged in, you can update certain information such as your name, email, and password.
Forums & Other Interactive Services
The Site/Apps may include discussion forums or other interactive areas or services, including chat rooms, bulletin boards, message boards, online hosting or storage services, or other areas or services in which you or other users create, post or store any content, messages, comments, reviews, materials or other items on the Site/Apps (“Interactive Areas”). If you use an Interactive Area, you should be aware that these areas may be viewable by others. We are not responsible for Personal Information you submit in connection with the Interactive Areas, nor are we responsible for how others might use that information, including to send you unsolicited messages. Interactive Area postings may be retained indefinitely.
Children
Consistent with the Children’s Online Privacy Protection Act (COPPA), Sulos does not knowingly request or collect Personal Information from children under the age of 13. If Sulos becomes aware that an individual under age 13 has provided information to Sulos via the Site or App, we will make commercially reasonable efforts to delete such information from our databases.
Third-party Links, Sites and Advertisements
We may link to websites, including those of our subsidiaries and third-party content providers, that have different privacy policies and practices from those disclosed here. We assume no responsibility for the policies or practices of such linked sites, and encourage you to become acquainted with them prior to use. Sulos expressly disclaims any and all liability for the actions of third parties, including but without limitation to actions relating to the use and/or disclosure of Personally Information by third parties.
Applicable Law
This Privacy Policy shall be governed by the laws of the State of Texas, without regard to conflicts of law principles. Any action relating to this Privacy Policy must be filed and maintained in a state or federal court of competent jurisdiction located in Texas, and each User consents to exclusive jurisdiction and venue in such courts for such purpose. Sulos makes no representation that this Privacy Policy and such practices comply with the laws of any country outside the United States.
Changes to This Privacy Policy
We reserve the right to change the terms of this Privacy Policy at any time. When we make changes, we will revise the “Last Updated” date at the top of the policy. If there are material changes to this statement or in how Sulos will use your Personal Information, we will notify you by prominently posting a notice of such changes here or on our home page, or by sending you an email. We encourage you to review this policy whenever you visit one of our sites.
